Community Patient Registration
- New community patients must complete the registration packet (click the button below). It is preferable that this is done prior to booking and arriving for your first appointment to avoid delays in your check-in process and to keep our provider schedule on time. Please be advised patients that receive primary care services at Potomac Pediatrics do not need to complete the new patient registration form.
- Community patients may self schedule with POP NOW using the Schedule Now link at the top of the website once you’ve completed the registration forms.
- Community patients that are not primary care patients of Potomac Pediatrics will be referred for follow-up care with their own PCP.
- POP NOW will send encounter notes to the PCP so please be prepared to provide this information upon check-in if you have not already completed the registration forms.
- POP NOW does not provide medical advice without an office visit.

Proxy Consent Authorization
To authorize another caretaker to bring your child into the office and to make decisions on your behalf please click below to complete the consent form Please note, this form MUST be completed if a parent is not bringing the child into the office. Please call us if you have any questions.
HMO Insurance Notice (Applicable to Community Patients only)
We accept all the same insurance as Potomac Pediatrics except for HMO plans. A list of these plans can be found at potomacpediatrics.com/billing-insurance. Community patients with an HMO plan must either be self-pay or provide a referral from their PCP prior to being seen. As a courtesy for the first visit, the patient may elect to be self-pay at the time of service and provide a referral after the visit. The referral must be provided within 7 days of the office visit. Referrals should be sent to billing@apparentmgmt.com. A refund will be processed back to the patient once the claim has been processed by the insurance company. The refund amount will be determined based on the amount of patient responsibility determined on the claim.
